Wegovy and cardiovascular health: what the evidence says

Wegovy is a prescription-only medicine containing semaglutide. It is best known as a weight management treatment, but it also has a UK cardiovascular risk reduction indication for certain adults with established cardiovascular disease and overweight or obesity.

What is Wegovy?

Wegovy is the brand name for semaglutide solution for injection in a pre-filled pen. Semaglutide is similar to glucagon-like peptide-1, also known as GLP-1, a naturally occurring hormone involved in appetite regulation.

Wegovy works by acting on receptors in the brain that help control appetite. This can help some people feel fuller, feel less hungry and reduce food cravings. It should be used with a reduced-calorie diet and increased physical activity.

Wegovy is a prescription-only medicine. It should only be used by the person it has been prescribed for and in line with the Patient Information Leaflet and prescriber instructions.

What cardiovascular conditions is this relevant to?

Cardiovascular disease is a broad term for conditions affecting the heart or blood vessels. It is often linked to fatty deposits building up inside arteries, which can restrict blood flow and increase the risk of a clot.

Examples include:

  • coronary heart disease
  • heart attack
  • stroke
  • peripheral arterial disease, where blood flow to the limbs is reduced

Risk factors for cardiovascular disease include high blood pressure, high cholesterol, smoking, diabetes, physical inactivity, unhealthy diet, overweight and obesity, family history and age.

Wegovy’s cardiovascular risk reduction evidence is not a general claim for everyone. It applies to a defined group: adults with established cardiovascular disease and overweight or obesity, in addition to standard care, diet and physical activity.

What is Wegovy approved for in relation to heart disease?

The UK Summary of Product Characteristics states that Wegovy is indicated as an adjunct to a reduced-calorie diet and increased physical activity to reduce the risk of major adverse cardiovascular events in adults with established cardiovascular disease and either obesity or overweight with a BMI of at least 27 kg/m².

Major adverse cardiovascular events, often shortened to MACE, usually refers to a composite endpoint including:

  • cardiovascular death
  • non-fatal myocardial infarction, also known as a non-fatal heart attack
  • non-fatal stroke

This does not mean Wegovy prevents all heart disease, treats an acute heart attack, replaces standard cardiovascular medicines, or is suitable for everyone with cardiovascular risk factors.

What did the SELECT trial find?

The SELECT trial was a large randomised clinical trial that looked at semaglutide 2.4 mg once weekly in adults with established cardiovascular disease and overweight or obesity, but without diabetes.

The trial compared semaglutide with placebo, both alongside standard care. The main endpoint was a first major adverse cardiovascular event, defined as cardiovascular death, non-fatal heart attack or non-fatal stroke.

In the Summary of Product Characteristics, the SELECT results report:

  • 569 of 8,803 people in the semaglutide group had a primary cardiovascular endpoint event
  • 701 of 8,801 people in the placebo group had a primary cardiovascular endpoint event
  • the hazard ratio was 0.80 with a 95% confidence interval of 0.72 to 0.90

This means semaglutide reduced the relative risk of the composite cardiovascular outcome by about 20% compared with placebo in this trial population.

It is important to interpret this carefully. The benefit was shown in people like those enrolled in the trial: adults with established cardiovascular disease and overweight or obesity. It should not be assumed to apply in the same way to people without established cardiovascular disease or to people who do not meet the licensed criteria.

Does Wegovy reduce the risk of heart attack or stroke?

In SELECT, the primary composite outcome of cardiovascular death, non-fatal heart attack or non-fatal stroke was lower with semaglutide than placebo.

The Summary of Product Characteristics also reports individual cardiovascular outcomes. Fatal or non-fatal myocardial infarction occurred less often in the semaglutide group than in the placebo group. Fatal or non-fatal stroke was numerically lower, but the confidence interval crossed 1. Cardiovascular death was also numerically lower, but superiority was not confirmed for cardiovascular death as an individual endpoint.

That distinction matters. The strongest evidence is for the combined MACE endpoint in the studied population, not a guarantee that Wegovy will prevent any individual event in any one person.

Does Wegovy help the heart because of weight loss?

Weight loss is likely to be part of the overall picture, because overweight and obesity are linked with several cardiovascular risk factors, including high blood pressure, type 2 diabetes and abnormal blood lipids.

However, the exact mechanism by which Wegovy reduces cardiovascular risk has not been established. The effect may involve multiple pathways, including weight reduction, blood pressure changes, metabolic effects, inflammation-related pathways or other mechanisms still being studied.

For patients, the practical point is that Wegovy should not be viewed as a standalone heart treatment. It is used alongside diet, physical activity and standard cardiovascular care where clinically appropriate.

What does NICE say?

NICE technology appraisal guidance TA1152, published in May 2026, recommends semaglutide as an option for reducing the risk of major adverse cardiovascular events in adults with established cardiovascular disease and overweight or obesity, within its marketing authorisation.

NICE states that semaglutide can be used alongside a reduced-calorie diet and increased physical activity, up to a maintenance dose of 2.4 mg once weekly, in adults with established cardiovascular disease and a BMI of at least 27 kg/m².

NICE also notes that standard care for established cardiovascular disease includes lifestyle changes and medicines aimed at reducing the risk of major adverse cardiovascular events.

Who may be considered for Wegovy for cardiovascular risk reduction?

A prescriber may consider Wegovy for cardiovascular risk reduction where a person meets the licensed criteria. This includes adults with:

  • established cardiovascular disease
  • overweight or obesity, with BMI at least 27 kg/m²
  • a clinical decision that semaglutide is appropriate alongside diet, physical activity and standard care

Established cardiovascular disease may include a previous heart attack, previous stroke or peripheral arterial disease, depending on the clinical criteria used.

Eligibility is not based on weight alone. A person’s medical history, current medicines, cardiovascular status, side effect risk, pregnancy status, diabetes status and other conditions all need to be reviewed.

What are the main side effects of Wegovy?

Like all medicines, Wegovy can cause side effects, although not everyone gets them.

Very common side effects reported in the Patient Information Leaflet include:

  • headache
  • nausea
  • vomiting
  • diarrhoea
  • constipation
  • stomach pain
  • feeling weak or tired

Common side effects include:

  • dizziness
  • indigestion or upset stomach
  • burping
  • wind
  • bloating
  • gastritis
  • reflux or heartburn
  • gallstones
  • hair loss
  • injection site reactions
  • changes in taste
  • low blood sugar in patients with diabetes

Gastrointestinal side effects can sometimes lead to dehydration, especially if vomiting or diarrhoea is persistent. This is particularly important for people with kidney problems or people taking medicines where dehydration could cause complications.

Serious side effects and urgent warning signs

Seek urgent medical help if you experience symptoms of a severe allergic reaction, such as:

  • difficulty breathing
  • swelling of the face, lips, tongue or throat
  • difficulty swallowing
  • wheezing
  • fast heartbeat
  • feeling faint, dizzy or weak

Stop using Wegovy and seek urgent medical help if you develop severe, persistent stomach pain, with or without nausea and vomiting. This may be a sign of acute pancreatitis, which can be serious.

Speak to a doctor promptly if you develop symptoms that could suggest gallbladder problems, such as severe upper abdominal pain, fever, yellowing of the skin or eyes, or pale stools.

People with diabetes should also seek advice if they experience signs of low blood sugar, especially if they take insulin or a sulfonylurea. Symptoms may include sweating, shaking, hunger, headache, fast heartbeat, confusion, weakness or visual changes.

Cautions, contraindications and interactions

Do not use Wegovy if you are allergic to semaglutide or any of the other ingredients listed in the Patient Information Leaflet.

Before using Wegovy, speak to a pharmacist, GP or prescriber if you:

  • have diabetes, especially if using insulin or a sulfonylurea
  • have diabetic eye disease or changes in vision
  • have ever had pancreatitis
  • have kidney problems or are at risk of dehydration
  • have significant gastrointestinal symptoms
  • are due to have surgery or a procedure requiring general anaesthesia or deep sedation
  • are pregnant, planning pregnancy or breastfeeding
  • take other injected medicines or other GLP-1 receptor agonists

Wegovy may delay gastric emptying and may potentially affect the absorption of some oral medicines. The Summary of Product Characteristics reports no clinically relevant drug-drug interactions with the evaluated medicines, but patients should still tell their prescriber or pharmacist about all medicines they take, including over-the-counter medicines and supplements.

Wegovy should not be used during pregnancy. If planning pregnancy, the Patient Information Leaflet advises stopping Wegovy at least two months beforehand. It should not be used while breastfeeding.

Wegovy does not replace standard heart disease care

People with cardiovascular disease are often prescribed medicines such as antiplatelets, statins, blood pressure medicines or diabetes medicines, depending on their individual condition.

Wegovy should not replace these treatments unless a prescriber specifically advises otherwise. Cardiovascular risk reduction usually requires a broader plan, which may include:

  • stopping smoking
  • blood pressure control
  • cholesterol management
  • diabetes or pre-diabetes management
  • physical activity appropriate to the person’s health status
  • diet and weight management support
  • taking prescribed cardiovascular medicines correctly
  • attending follow-up appointments and monitoring

When to speak to a pharmacist or GP

Speak to a pharmacist, GP or prescribing clinician if:

  • you have heart disease and want to understand whether Wegovy may be suitable
  • you have had a heart attack, stroke or peripheral arterial disease and are living with overweight or obesity
  • you are taking insulin, sulfonylureas, blood pressure medicines, anticoagulants or other long-term medicines
  • you are getting persistent nausea, vomiting, diarrhoea or constipation
  • you are struggling to eat or drink enough
  • you have symptoms of dehydration, such as dizziness, reduced urination or feeling unusually weak
  • you notice changes in vision, especially if you have diabetes
  • you are due to have surgery or an endoscopy under anaesthesia or sedation
  • you are pregnant, breastfeeding or planning pregnancy
  • you are unsure how to use the pen, store it or dispose of needles safely

Seek urgent medical help for severe allergic symptoms, severe persistent abdominal pain, symptoms of pancreatitis, or symptoms that could suggest a heart attack or stroke.

Is Wegovy used for heart disease?

Wegovy is not used to treat an acute heart attack or stroke.
