What is Foundayo?

Foundayo tablets contain orforglipron and are a prescription-only oral medicine used for weight management in eligible adults. Foundayo must be used alongside a reduced-calorie diet and increased physical activity.

Orforglipron acts similarly to a natural hormone called glucagon-like peptide-1 (GLP-1), which regulates blood sugar, appetite, and metabolism. It works on receptors in the brain involved in appetite control, helping you feel fuller for longer, experience fewer food cravings, and reduce your overall food intake.

In clinical studies for Foundayo, adults receiving the highest maintenance dose of 17.2 mg once daily, alongside a reduced-calorie diet and increased physical activity, had an estimated average reduction in body weight of 12.4% after 72 weeks. This result relates to the highest maintenance dose, not the starting dose. Individual responses vary, and not everyone will lose the same amount of weight.

Who is Foundayo intended for?

Foundayo tablets are licensed for weight management in adults with an initial body mass index of:

• 30 kg/m² or above, which is classified as obesity; or
• 27 kg/m² to below 30 kg/m², which is classified as overweight, when at least one weight-related health problem (such as high blood pressure or high cholesterol) is also present.

Foundayo tablets are not recommended for children or adolescents under 18 years of age, as their safety and effectiveness have not been established in this age group.

Foundayo starting dose

Foundayo 0.8 mg is the once-daily starting dose used during the first month of treatment to allow your body to adjust gently. The dose is then usually increased gradually:

• Month 1: 0.8 mg once daily.
• Month 2 onwards: 2.5 mg once daily, with potential further increases every 30 days.

Each dose level is normally used for at least one month (30 days). Dose increases should only be made in accordance with your prescribed dosing schedule, depending on your response and tolerance, up to a maximum maintenance dose of 17.2 mg daily.

Do not take more than one tablet per day to obtain the effect of a higher dose.

How to take Foundayo tablets

Take one Foundayo tablet once daily:

• Swallow the tablet whole.
• Do not split, crush, or chew the tablet.
• No food or water restrictions: Unlike other oral GLP-1 medicines, Foundayo can be taken at any time of day, with or without food or water, making it easy to fit seamlessly into your routine.

Taking your tablet at the same time each day can help you remember to take it.

Active ingredient: Each Foundayo 14.5 mg film-coated tablet contains orforglipron calcium equivalent to 14.5 mg orforglipron.

Other ingredients: The tablet core contains microcrystalline cellulose, crospovidone, copovidone, sodium carbonate anhydrous and magnesium stearate. The film coating contains poly(vinyl alcohol), titanium dioxide, macrogol, talc, iron oxide yellow (E172) and iron oxide black (E172).

Additional information: This medicine is essentially sodium-free.

Take one Foundayo 14.5 mg tablet by mouth once a day, at any time of day, with or without food, and without needing to drink extra water. Swallow the tablet whole. Do not break, crush or chew it.

14.5 mg is reached gradually. Treatment starts at 0.8 mg once daily and is increased, no more than once every 30 days, through 2.5 mg, 5.5 mg and 9 mg, before reaching 14.5 mg. Depending on your response and tolerability, your prescriber may increase your dose further to the maximum of 17.2 mg once daily.

If you forget to take a dose, take it as soon as you remember that day. Do not take an extra tablet to make up for a missed dose, and take no more than one tablet in a day.

Store Foundayo in its original packaging, as the tablets are sensitive to light.

• Do not take Foundayo if you are allergic to orforglipron or any of the other ingredients in the tablet.
• Not recommended during pregnancy, as the effects on an unborn baby are not known. Use effective contraception and stop treatment at least 3 weeks before trying to conceive. Foundayo may reduce the effectiveness of oral contraceptives, so barrier contraception (such as condoms) is recommended for 30 days after starting treatment or after any dose increase.
• Not recommended while breastfeeding.
• Tell your prescriber before starting if you have ever had pancreatitis, gallbladder problems, severe digestive problems, diabetic eye disease, or severe heart failure.
• If you also take insulin or a sulfonylurea, your doctor may lower that dose to reduce the risk of low blood sugar (hypoglycaemia).
• If you take medicine to lower your blood pressure, tell your doctor before starting Foundayo.
• Nausea, vomiting or diarrhoea can cause dehydration. Make sure you drink enough fluids while taking Foundayo, as dehydration can affect your kidneys.
• If you have surgery planned, tell your medical team that you are taking Foundayo.
• Tell your prescriber about all other medicines you take. Some, such as clarithromycin, itraconazole or ritonavir, can increase orforglipron levels, while others, such as carbamazepine, rifampicin or St John's wort, can make it less effective.
• Not licensed for use in anyone under 18.
• Foundayo is a new medicine subject to additional monitoring. Report any side effects you experience to your doctor or pharmacist.

Gastrointestinal side effects such as nausea and diarrhoea are usually mild to moderate and tend to lessen over time.

Very common (may affect more than 1 in 10 people): nausea, constipation, diarrhoea, vomiting, indigestion, abdominal pain, and low blood sugar when Foundayo is taken with insulin.

Common (may affect up to 1 in 10 people): low blood sugar when taken with a sulfonylurea, dizziness, headache, a fast heartbeat, low blood pressure, bloating, burping, heartburn, wind, gallstones, hair loss, tiredness, and raised pancreatic enzyme levels on blood tests.

Uncommon (may affect up to 1 in 100 people): low blood sugar without insulin or a sulfonylurea, an altered sense of taste, abnormal skin sensations such as tingling or numbness, and acute pancreatitis.

Rare or not known: changes in vision, bowel obstruction, severe constipation, serious allergic reactions, inhaling stomach contents into the lungs during anaesthesia or sedation, and kidney problems.
